Description (Service)

An initiative of the Ontario Complex Special Needs Strategy 

  • supports families who are involved with multiple services
  • provides children and youth with multiple and/or complex special needs and their families with a high level of seamless and family-centred care
  • responsible for planning and coordinating child services, in collaboration with the family and other services and supports

Office Mon-Fri 9 am-5 pm

Areas Served Halton Region
Eligibility Ages: 18 year(s) and under or 21 and younger and still enrolled in school
  • resident of Halton
  • currently being supported by 1 active service provider in addition with school supports (not including wait lists, physicians, private psychologists/psychiatrists or others that cannot attend and/or participate in regular coordination meetings pertaining to their role with the family)
  • a Service Coordination meeting has occurred (date required) with the family and other providers and a clear identified goal of youth/caregiver is identified (action plan or notes from coordination meeting helpful but not required)
  • the youth and caregiver have a clear understanding of Coordinated Service Planning and their requirement of active participation
  • the family is willing to communicate and share information and reports with all approved community supports/partners
